You must:

Instructions

  2. Enter the store number found on your receipt. If the store number is not found, enter the state where you were and select the specific store. Once you’re done, click Next.
  3. Enter your date and choose whether you dined in, called in, or ordered online.
  4. Select the time period for your visit.
  5. Answer questions about your recent experience. Many questions only allow one answer, while some others allow multiple answers. Please carefully read the instructions. A text box may be available to provide additional information.
  6. Once you have completed the questionnaire, you will receive a redemption code. This code will be written on the receipt. Take it with you to redeem at your next visit.
