How to Take the Charley’s Guest Satisfaction Survey

  • Access to the Internet and a computer
  • Learn Spanish or English.
  • Have your recent Charley’s receipt that contains an invitation for the survey.

  2. Click the appropriate hyperlink to view Spanish and Arabic versions.
  3. Click on the Start button to enter your store number (5 digits), date, and time from receipt. You will need to take your time in finding this information.
